A 60-year-old male patient with low back pain and right lower limb pain and numbness was hospitalized for one month. Lumbar decompression and interbody fusion was performed through the right intervertebral foramen of L4/5. The cortical bone of L4/5 left facet joint and transverse process were removed during operation. during the operation, and preventive decompression was performed. The operation was successful, the postoperative recovery was good, and no contralateral root symptoms occurred. 4a, Preoperative MRI showed severe stenosis of the L4/5 bilateral intervertebral foramen. 4b, The right lamina and facet joint of L4/5 were removed. The cortical bone of L4/5 left facet joint and transverse process were removed
