A 48-year old female suffered from progressive numbness and weakness in the left upper limb for 1 years. Sagittal X-ray (a) and sagittal reconstructed CT images of the cervical spine (b) showed os odontoideum and subluxation at the atlanto-axial joint. Posterior atlantoaxial reduction, fixation and bone graft fusion with intraoperative skull traction under general anesthesia were performed. Satisfactory reduction and fixation were achieved after surgery (c). Sagittal reconstructed CT images of the cervical spine (d) at 1-year follow-up confirmed good fusion at the atlanto-axial joint
