A 63-year-old male patient with multilevel, mixed-type ossification of the posterior longitudinal ligament. Preoperative computed tomography scan showed ossification of the posterior longitudinal ligament involving C2-T1 (a). Preoperative magnetic resonance image showed the pathological extent involving 6 intervertebral levels (b). He accepted crossing the cervicothoracic junction fusion from C2 to T1 and acquired sufficient decompression from C2 to C7 (c, d). Computed tomography scan of 12 month after surgery showed progression of ossification of the posterior longitudinal ligament (e).
